The most serious documented side effect of Kombiglyze XR is lactic acidosis, which can occur due to metformin accumulation. The risk increases with conditions such as dehydration, sepsis, excess alcohol intake, hepatic impairment, renal impairment, and acute congestive heart failure. Associated symptoms include malaise, respiratory distress, myalgias, increasing somnolence and nonspecific abdominal distress. These adverse effects require immediate hospitalization. Lactic acidosis is associated with cardiovascular collapse (shock), acute congestive heart failure and acute myocardial infarction. The risk is very small, however.

Symptoms such as malaise, myalgias, respiratory distress, increasing somnolence and nonspecific abdominal distress could point to lactic acidosis and since this is a most serious side effect that could lead to death without rapid treatment, a doctor should be consulted immediately.

Vitamin B12 deficiency is a risk of using Kombiglyze XR. Reported adverse effects are urticaria, facial edema, headache, nasopharyngitis, upper respiratory tract infection, urinary tract infection, diarrhea and nausea/vomiting.
